Before filling out a registration form, check you are in our catchment area.

To register with the surgery

To register with Montgomery Medical Practice any new patients would need to present at the surgery and complete registration forms for every member of the family. You will be offered a new patient check appointment at a time convenient for yourselves for every adult family member.  At this point any new patients would be provided with information and services that the practice can offer.

Even though patients are registered with a particular doctor, patients can opt to see their preferred practitioner.  Please ask by name for the practitioner when making an appointment. Please note that if the doctor is on annual leave another doctor will be offered.

Please make sure you complete and return the patient registration form and questionnaire that applies to you. Without both these documents we are unable to accept the registration.

Disabled Access

The practice has two disabled car parking spaces by the practice.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access, please collect a proxy access form from reception from 8am to 6pm.

Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.

Text Messaging

Montgomery Medical Practice would like to offer you the ability to receive text messages for health promotion and for appointment reminders.

The SMS service should not be solely relied upon, as the responsibility of attending and cancelling appointments still rests with you, but we hope this will make things easier.

Messages are generated by an NHS secure service; however, they are transmitted over a public network to a personal phone.

The practice will never transmit any information that would enable an individual patient to be identified or specifically which tests they have had.
